Director of Palliative Care

We are seeking a highly motivated and dynamic leader to be our Director of Palliative Care program at Senior Health.  The Director of Palliative Care is responsible for the day-to-day operations of the Palliative Care program consistent with the mission, vision, and values of the organization. They are also responsible for the program’s operational success including: staffing, fiscal responsibility, customer satisfaction and quality care and supporting the continuum of care that has been established.
JOB SUMMARY: The Director of Palliative Care is a dynamic leader with a proven history of leading a team of clinicians with attention to customer satisfaction, fiscal responsibility, quality control, employee education and retention. She/he supervises the palliative care provided by nurse practitioners, nurses, social workers, and office staff under the direction of Medical Director and Administrator. The Director of Palliative Care is available at all times during operating hours (or appoints a similarly qualified alternate) and oversees the daily operations of the program.
QUALIFICATIONS:
  • Three to five years of experience in health care/home care, preferred.
  • Two years’ experience in a supervisory position preferred.
  • Knowledge and ability to apply community health principles and practices.
  • Knowledge of Agency policies and procedures.
  • Ability to supervise, guide and develop skills and performance of service personnel.
  • Ability to exercise independent judgment.
  • Ability to work with individuals.
  • Ability to enlist cooperation of many people in furthering a program.
  • Ability to assist in evaluating personnel per Agency policy.
  • Monitors new employee orientation and expectations
  • Ability to deal effectively with high levels of stress.
RESPONSIBILITIES:
  • Participates in developing standards which ensure safe and therapeutically effective service to patients and families. Has joint responsibility with the Administrator for seeing that standards are met.
  • Participates in developing objectives for the program.
  • Is responsible for seeing that objectives are implemented.
  • Consults with the Administrator to determine a staffing pattern which will accomplish stated objectives and promote maximum level of utilization of health personnel.
  • Assists Administrator in recruiting, hiring, evaluating and terminating service personnel.
  • Oversees orientation of new employees according to agency orientation program.
  • Displays a willingness to support the policies and procedures and uses appropriate channels for change of such policies.
  • Participates in establishing the functions and qualifications for each service position, and coordinates patient care and referrals.
  • Assures timely attention to requests by customer (Optum) tracking metric outcomes and reporting these metrics regularly.
  • Participates in coordinating Agency’s services with other community agencies or partners.
  • Participates in studies and research and other administrative functions as assigned.
  • Serves as a role model for all colleagues by setting an example of high standards in dress, conduct, cooperation, and job performance.
  • Observes confidentiality and safeguards all patient related information.
  • Accepts responsibility for regular attendance and punctuality and fulfills job requirements without regard to time involved.
  • Serves as a resource person to employees.
  • Develops a cooperative relationship and communicates effectively and professionally with practitioners.
  • Investigates and reports any problem relating to patient care or conditions which might harm the patient and/or an employee’s well-being.
  • Immediately reports any accident, incident, lost articles or unusual occurrence to the Administrator.
  • Attends pertinent continuing education programs other than routine in-services and shares information with staff.
  • Works with Administrator in identifying budgetary requirements and determining appropriate use of allocations.
  • Arranges 24-hour/day, seven (7) days/week on-call nursing coverage.
  • Plans for and supervises the Agency.
  • Conducts staff in-services, staff meetings and IDT meetings.
  • Ensures that all necessary supplies and equipment are available.
  • Is normally available at all times during operating hours and after operating hours in accordance with on call rotation; may designate a qualified temporary replacement if he/she will not be available.
  • Makes Case Manager assignments.
  • Other Duties as assigned.
