Rehabilitation Therapy

Our team of rehabilitation specialists works with our patients and their families to restore residents to the highest possible level of independence. Our physical, occupational and speech therapists help residents to increase their strength and relearn basic living skills that may enable them to return home or to lesser levels of care.

The therapy team is comprised of a friendly and dynamic group of PTs, PTAs, OTs, COTAs, and SLPs. Our team has accumulated a wide range of experience in several settings including:

Family and Social Support Services:

An active mind and positive spiritual outlook promote overall wellness. Our programs consist of activities based on the interests and abilities of each person. Our recreation therapists and social workers consider the physical, emotional, intellectual, and spiritual needs of each patient. Van transportation allows our patients to remain active in the community.

Who Pays?

Services are paid in a variety of ways. These typically include:

  • Medicare: After a three day qualifying hospital stay, Medicare will pay for 20 days of a skilled nursing care facility at full cost and the difference between a daily co-pay and the actual cost for another 80 days.
  • Medicaid: Medicaid is a welfare program jointly funded by the federal government and individual states. It is largely administered by the states. Check with your Medicaid office to determine your eligibility and what will be covered.
  • Insurance: There are many commercial insurance carriers that have programs to help cover the cost of a Skilled Nursing Home stay. Please check with various carriers to determine which plan best meets your needs.
  • Private: If there are no insurance options available, then paying privately is an option for many residents.

What does Medicare and Medicaid cover?

Medicare has two parts. Part A (Hospital Insurance) pays some of the costs of hospitalization and limited skilled nursing care. Part B (Medical Insurance) primarily covers doctor’s fees, hospital outpatient services, and certain related services. Medicare does not cover long-term nursing facility care.

Medicare will assist eligible persons for the first 100 days of a nursing home stay. Medicare will pay 100% of the cost of services for the first 20 days. For the remaining 80 days of eligibility, the resident will pay a $157.00 daily co-pay and Medicare will pay the remaining balance. Please talk to a facility representative regarding eligible Medicare stays.

Medicaid is a state run program that shares in the cost of care with the resident. Upon determining whether the individual is eligible (done by a state Medicaid eligibility worker), the state will determine how much of the cost will be covered under the program. For further details regarding the Medicare or Medicaid programs, please talk to a facility representative.

Can I have a private room?

Yes, depending on availability of the private rooms.

Do you have licensed nurses on staff?

We have both Registered Nurses and Licensed Practical Nurses staffed in the facility 24 hours each day.

Can I leave for activities outside the facility while I am here?

People who are here on a Medicare stay for rehabilitation are only able to leave the facility for doctor appointments or for an approved therapeutic leave. Long-term residents can leave the facility by simply checking out with a nurse. Please talk to the staff about taking a leave of absence.

What is the ratio of CNAs and Nurses to patients?

Our staffing ratios change depending on the overall facility census. However, we always maintain ratios that allow us to provide high quality nursing care to our residents. We are constantly monitoring our staffing ratios and making changes as needed.

Do you offer physical, occupational and speech therapy?

The facility does offer physical, occupational and speech therapy services.

How often will I receive therapy services?

Residents receive therapy based on the recommendation of a physician and the facility therapy staff. The frequency of therapy treatments is determined on a case-by-case basis and qualifying needs.

What is the quality of food like?

Rocky Mountain Care is committed to serving the highest quality food to their residents. Many of our staff and residents families frequently dine in the facility. Although many of our residents are on special diets, all of our food is appealing and appetizing. We invite you to join us for meals on occasion.

How often will my loved one receive a bath/shower?

Typically, residents receive a shower 2-3 times a week. However, daily showers are available upon request.

What time are visiting hours?

Family members are welcome to visit residents any time of the day. Visitors must be respectful when other residents are sleeping, or needing privacy.

Who distributes medication?

A Registered Nurse or a Licensed Practical Nurse will administer all medications.

Do you have a doctor at the facility?

Federal law requires that all those residing in a skilled nursing facility be under the care of a medical doctor. Rocky Mountain Care has a Medical Director on staff. Although he/she is not located on the premise on a full-time basis, he/she does frequent our facilities often.
